Young Adult Transition Services

The transition to adulthood can be complex for many adolescents. This package will help focus on goals and tools your child can use to build independence, organization, self-advocacy and other skills needed to meet their goals. These services are appropriate for those planning to transition to:
  • a secondary education program
  • traditional college
  • a day program
  • move into a group living situation
  • needs vocational training
  • would like to enter the workforce,

Includes:
  • Initial consultation to learn about your child and their future goals
  • 4 sessions to develop 1-3 specific goals and objectives 
  • Written plan for ways to work on each goal within the home and community

  • Additional follow-up sessions can also be purchased if needed
